Membership plan fees may be paid by a credit or debit card (AMEX, Discover, MasterCard, Visa). Your office may opt to allow patients to pay for their plan via cash, check or another third-party lender. Ask your office for approved payment methods.

Upon enrollment, you will be sent a conformation email that will provide your dentist’s office information to schedule your next hygiene appointment. NOTE – requested appointment time is not always guaranteed. The office staff will contact you with a confirmed appointment time. You may also contact your dentist’s office directly to schedule your next office visit.
Your office will have your plan discounts on file. Restrictions may apply. Contact your office or ask about your plan discounts with your office’s treatment coordinator or staff.
A periodontal membership is for patients with periodontal disease and who need periodontal maintenance and cleanings. If you are unsure of the plan you should purchase for yourself or family member, contact your office.
Plan benefits are valid for one year from date of enrollment. If you are unable to make your upcoming hygiene appointment, contact your dental office to reschedule.
Yes, you may add your family members (those whom reside in your household) on your plan. Contact your dental office to add your family member to your plan.
Your dental savings plan is an agreement made between you and your provider and may only be used/claimed at the office in which it was purchased from. If you are relocating/moving, call your dental office to cancel your plan.
Upon enrollment you will receive access to your own portal that allows for you to review and modify your plan.
In your portal, you can modify your billing information. If you have trouble accessing your portal, contact your dentist’s office.
Your portal allows for you to cancel your plan. You can also contact your dentist’s office directly to cancel or modify your plan.
You have 30 days to cancel your plan to be eligible for a full refund (lesser of any processing fees). No services must have been used/claimed. After 30 days, it is up to your offices discretion to determine refund eligibility. Contact your dentist’s office with any questions regarding your plan.
Your dentist may have various payment options – such as monthly or annual. Plan may need to be paid in full or over a series of payments. Review your office’s enrollment page or contact your office directly for questions regarding payment options.
Your office may have options to pay for up to two years in advance. Contact your office for more information for paying ahead.
Upon enrollment, the patient will need to provide their name, billing information and email (for online account creation and reminders).
